On February 27th The Memorial Foundation received a grant from The Rite Aid Foundation in the amount of $5,000!
This grant opportunity will support Memorial Hospital’s Tomando Control de su Salud (Taking Control of Your Health), which is a chronic disease self-management program that primarily targets high risk, low-income Latino adults and families. Program participants learn to design and practice healthy diet, exercise and stress management routines, and are given tools to continue these behaviors at home to benefit the entire family. The goal is to help those with chronic diseases such as diabetes, hypertension, heart disease, obesity or arthritis better manage their symptoms, and to help those who have risk factors for disease stay healthy to reduce the likelihood of developing a chronic illness.
Health education is of vital importance to the Latino community, particularly to recent immigrants. Lack of access to primary care, preventative care, and treatment contributes to health disparities for the Latino community. These critical health disparities lead to higher rates of chronic disease in minority communities, which significantly impacts the healthcare system.
